The Board may issue an initial license to an applicant who is currently registered with a national certifying body (ARRT, ARDMS, NMTCB, CCI, etc.). If you have already graduated and received your registry exam results or registry card, you will apply for an Initial license. If you are already licensed or practicing in another state and wish to practice in ND permanently or as a traveler, you will complete this application.
Do NOT send the background check forms/prints and payment to ND BCI directly, they MUST be sent to the Board office for processing. ND BCI will reject all forms/prints and payment if sent directly to them; everything will be returned back to you causing additional delays in the licensure process.
Note: If the background check results are clear and all documents have been received, the license may be issued.
Applications will be kept open for 6 months from date of application. The timeline should give you enough time to gather any required information and complete the background check. There are NO REFUNDS for not completing the application within 6 months.
The application will ask if you are currently licensed in another state. If you are licensed in another state, you will need to list the state(s) and license number(s). If you are licensed in multiple states or multiple modalities with some having state licensure and some not having licensure, please do not include the states without licensure in your endorsement application. Only list the states that you are licensed in.
Endorsement applicants also need to provide primary source verification. Please visit the Dept of Health or your State Licensure website and upload/save an online verification for that state. If you are licensed in multiple states, please upload a document for each state.
**Note: This is not a copy of your current license. It must come directly from the other state as a primary verification.**
If you are currently licensed and in good standing in another state/jurisdiction and are here on ND Military Orders, you may qualify to have the license fee waived. As defined in Federal HR 7939 effective January 5, 2023: you must provide military orders of assignment in ND.
