- The paper application is no longer accepted as most applications are online. The board will be accepting either Credit Card or Check/Money Order Payments. There will be a delay in processing the application when paying by Check/Money Order due to the delay in receiving payment. Applications will not be approved until payment has been received. Because of the mail delays mentioned below, the Board encourages you to make your payment online.
Do NOT send the background check forms/prints and payment to ND BCI directly, they MUST be sent to the Board office for processing. ND BCI will reject all forms/prints and payment if sent directly to them; everything will be returned back to you causing additional delays in the licensure process.
Note: If the background check results are clear and all documents have been received, the license may be issued.
Applications will be kept open for 6 months from date of application. The timeline should give you enough time to gather any required information and complete the background check. There are NO REFUNDS for not completing the application within 6 months.
Licensure by Endorsement
The application will ask if you are currently licensed in another state. If you are licensed in another state, you will need to list the state(s) and license number(s). If you are licensed in multiple states or multiple modalities with some having state licensure and some not having licensure, please do not include the states without licensure in your endorsement application. Only list the states that you are licensed in.
Endorsement applicants also need to provide primary source verification. Please visit the Dept of Health or your State Licensure website and upload/save an online verification for that state. If you are licensed in multiple states, please upload a document for each state.
**Note: This is not a copy of your current license. It must come directly from the other state as a primary verification.**
Military
If you are currently licensed and in good standing in another state/jurisdiction and are here on ND Military Orders, you may qualify to have the license fee waived. As defined in Federal HR 7939 effective January 5, 2023: you must provide military orders of assignment in ND.
Qualifications
- Active Military Orders of Assignment in North Dakota
- Actively practicing medical imaging for the last 2 years.
- Must provide employment for the last 2 years of active employment in medical imaging.
- Currently licensed and in good standing in another state/jurisdiction in which a medical imaging license is held.
- Agree to submit to our North Dakota authority.
Documents Required
- Active Military Orders of Assignment for North Dakota
- Must provide employment information for the last 2 years of active employment in medical imaging.
- Endorsement documentation from another state/jurisdiction in which a medical imaging license is held.
- Military Identification
- If Military spouse of active member status - appropriate proof are a military issued ID noting relationship to the military member and the orders of the active military member spouse.
Prior to beginning the application, please ensure you have all the correct documents.
- A copy of your current registry card(s) with a national certifying agency (ARRT, ARDMS, etc.). If your registry no longer issues registry cards, you must still send the board office a proof of primary source verification from your National Registry Website.
- If you answered "Yes" to any of the questions in the Personal Background History, an explanation and any relevant documentation as requested at the end of that section must be submitted with the application. If you cannot send a copy of the criminal judgment, please include that in your explanation.
- If you are currently licensed and in good standing in another state/jurisdiction and are here on ND Military Orders - please have all military documents ready to upload. **See above for list of documents**
- If applying for a license by endorsement you will need to request all the state(s) you hold or have held a license to send a verification of your license in those state(s). If the state(s) have online verification, send the website address and directions to verify your license in writing or by email.

During online application, please do not use the back button in your browser. To make a change to answers on a previous page or to review your application before submitting, please use the "Save for Later" button to save your application, log back in and access and review the previous pages. If at any time you wish to exit the process, please click the "Exit" button located at the bottom of the page.
If you will be away from your computer for any period of time after starting your online application, it is strongly advised that you click on the "Save for Later" button located at the bottom of each page to minimize the risk of losing the data that you have input into the system. You can then log back into the application later.
